Abstract Original Research
introduction : Spinal cord injury (SCI) interrupts electrochemical communication between the brain and the periphery, producing permanent or transient motor, sensory, and autonomic deficits. Research addressing this interruption has developed largely along three separate tracks: neuro-electronic bypass devices, regenerative bioengineering, and molecular/gene therapy. Pairwise combinations of two of these three tracks have already been tested in registered human trials and published clinical series (e.g., scaffold transplantation combined with epidural electrical stimulation, NCT03966794; biomolecule-functionalized regenerative scaffolds combined with cell therapy [39]) a correction added in this revision to an earlier, overstated claim of "little cross-track integration." What has not, to our knowledge, been reported is a single time-phased architecture combining all three tracks together with an adaptive closed-loop control principle.
Methods: This is a narrative, non-systematic synthesis. Sources (peer-reviewed articles, review papers clinical-trial registry entries, and one patent) were identified through general biomedical web search and search of PubMed/PMC, ClinicalTrials.gov, ScienceDirect, and Frontiers, restricted to material retrievable up to July 2026. No PRISMA screening, database deduplication, or formal risk-of-bias appraisal was performed; the review therefore has scoping-review rigor rather than systematic-review rigor, and findings should be read accordingly.
Results: Three trajectories were characterized. (1) Neuro-electronic bypass: cortical brain–spine interfaces have restored volitional walking in a chronically paralyzed human; local “electroceutical” bridges and epidural stimulation restore function without decoding cortical intent, at the cost of coarse, non-synaptic signal resolution. (2) Regenerative bioengineering: conductive hydrogel scaffolds bioprinted with neural stem cells guide axonal outgrowth across a transected cord in rodent models, improving hindlimb function, but require months and carry immunological and tumorigenic risk. (3) Molecular/gene therapy: chondroitinase ABC, anti-Nogo-A antibodies, and Rho-ROCK inhibitors reduce the glial-scar barrier to regeneration, with delivery stability remaining the principal translational obstacle. No study identified combined all three tracks within a single, time-phased system.
Conclusion: We propose, as an original synthesis rather than an established finding, a three-layer time-phased architecture (named PHASE-B in this review) in which (i) an immediate local electronic bridge restores function within days, (ii) the same electrode pair is embedded within a conductive bioprinted scaffold that simultaneously guides biological axon regrowth over weeks to months, and (iii) local chondroitinase ABC / anti-Nogo-A delivery lowers the glial-scar barrier during this window. We further propose a closed-loop control principle in which the electronic contribution is adaptively reduced as biologically generated signal amplitude increases, monitored longitudinally. This framework is explicitly theoretical and unvalidated; it is offered as a testable research roadmap, not a clinical recommendation.
