Abstract Original Research
introduction : Epilepsy is one of the most prevalent chronic neurological disorders worldwide, and accurate localization of epileptogenic zones plays a crucial role in optimizing therapeutic strategies, particularly in patients with drug-resistant epilepsy. Recent advances in neuroimaging and electrophysiological signal acquisition have enabled more precise structural and functional brain mapping. These novel techniques provide deeper insights into brain network alterations associated with epileptic activity. The objective of this study was to evaluate the effectiveness of advanced brain mapping approaches based on modern neuroimaging modalities and signal recording techniques in identifying epileptogenic regions in patients with epilepsy.
Methods: In this experimental study, 45 patients diagnosed with focal epilepsy (23 pediatric and 22 adult patients) were recruited. High-density electroencephalography (HD-EEG) was used to acquire electrophysiological data, while structural magnetic resonance imaging (MRI) and functional magnetic resonance imaging (fMRI) were employed to obtain neuroimaging data. Data preprocessing included artifact removal, signal filtering, source localization, and spatial co-registration between electrophysiological and imaging datasets. Functional connectivity metrics were extracted to analyze brain network organization. The resulting brain maps were compared with conventional clinical findings, routine EEG reports, and neuroimaging assessments performed by expert neurologists. Statistical analyses were conducted using appropriate descriptive and inferential tests to evaluate the accuracy and consistency of epileptogenic zone localization.
Results: The combined use of HD-EEG and fMRI significantly improved the accuracy of epileptogenic zone localization compared with conventional diagnostic methods. In 78% of the patients, a high level of concordance was observed between advanced brain mapping results and clinically identified seizure onset zones. Functional network analysis revealed significant disruptions in connectivity patterns within epileptic networks (P < 0.05). Furthermore, distinct differences were observed between pediatric and adult patients in terms of the spatial extent and organization of the affected brain networks, suggesting age-related variability in epileptic network dynamics.
Conclusion: The findings of this study demonstrate that advanced brain mapping techniques integrating neuroimaging and electrophysiological signal recording provide a powerful and reliable approach for localizing epileptogenic zones in patients with epilepsy. These methods offer enhanced diagnostic precision and have the potential to significantly improve clinical decision-making, surgical planning, and personalized treatment strategies, particularly in patients with refractory epilepsy. Incorporation of advanced brain mapping into routine clinical practice may lead to improved therapeutic outcomes and a better understanding of epilepsy-related brain network alterations.
